Some listings here will be wrong. The federal record is only as current as the last time a provider updated it, and a license can lapse between our checks. There are two ways to fix one.

Tell us

Use the correction form. Say which office and town, and what is wrong: closed, moved, new phone number, wrong name, or missing from the page. It is free and anyone can use it, whether you run the office or just know the town.

We can take a closed office off the page. A missing office, or a new name, address, or phone number, has to come from the federal record, because we do not publish details the record does not hold.

Fix it at the source

If the record is yours, you can update it yourself in NPPES, the federal system that holds it. Where we check a state's license list, holding a current license there is what keeps you listed, however old your federal record gets. Everywhere else, re-dating your federal record is what puts an office with an old record back on its town page. Either change reaches this site after the next monthly rebuild.

Asking to be removed

The listings are public federal records. If you would still like yours left off this site, send the form and say so.
